    • Proof of income - e.g., pay stubs; LES; tax return; food stamp, QUEST or other benefit documents.
    • Proof of identity
    • Your baby and/or children who need WIC
    • Your baby's and/or children's shot record
    • The referral form completed by your Health Care Provider or yourself. WIC can take your height, weight, and assess the need for a blood test.
